One of the most heartbreaking things I hear from women isn’t,

“I’m exhausted.”

It’s this:

“I don’t recognize myself anymore.”

These are women who are doing everything they’re supposed to do.

They’re eating well.

They’re exercising.

They’re taking supplements.

Many have even started hormone replacement therapy (HRT).

For a little while, things improve.

Then the brain fog returns.

The afternoon energy crash comes back.

The weight starts creeping up again.

And they begin asking the same question:

“What am I missing?”

Hormonal changes are absolutely part of the story during perimenopause and menopause. But they’re often not the entire story.

That’s why so many women continue to struggle, even after trying treatments that initially seemed to help.

When Symptoms Return, It’s Time to Look Deeper

Hormone replacement therapy has helped many women improve their quality of life, and for some, it’s an important part of their healthcare plan.

But one pattern has become increasingly common in my practice.

A woman starts HRT and finally feels hopeful. Her energy improves. Her sleep gets better. She starts thinking, “I’m getting my life back.”

Then, weeks or months later, the brain fog slowly returns. She’s exhausted again. The weight becomes harder to lose. Her mood feels less stable.

It’s frustrating.

Not because HRT has “failed,” but because hormones aren’t always the only thing contributing to these symptoms.

Sometimes they’ve been covering up deeper imbalances that still need attention.

That’s why I believe it’s so important to ask a different question.

Instead of asking, “Do I need more hormones?”

We also need to ask, “What else is going on in my body?”

Your Symptoms Are Real, Even If Your Lab Work Looks Normal

One of the hardest things a woman can hear is,

“Everything looks normal.”

Especially when she knows something has changed.

She’s forgetting appointments.

Walking into rooms without remembering why.

Struggling to stay focused at work.

Feeling overwhelmed by tasks she used to handle with ease.

Routine blood work is incredibly valuable, but it doesn’t always provide every answer.

Sometimes the clues are hiding in places that aren’t routinely investigated.

That’s why I never want women to believe their symptoms are “just part of getting older” simply because a standard blood test didn’t reveal an obvious explanation.

You know your body better than anyone.

If something feels different, it’s worth continuing the conversation.

There Isn’t One Root Cause for Every Woman

One of the biggest misconceptions about midlife health is that every symptom comes down to low estrogen.

In reality, every woman has her own story.

Her own health history.

Her own genetics.

Her own biology.

That’s why two women can have nearly identical symptoms but need completely different treatment plans.

For one woman, unstable blood sugar may be contributing to inflammation, fatigue, cravings, and stubborn weight gain.

Another may have significant cortisol imbalances after years of chronic stress, making it difficult to sleep, think clearly, or recover from everyday demands.

For someone else, the biggest contributor could be hidden inflammation in the gut.

These underlying issues often don’t disappear simply because hormones are replaced.

That’s why a personalized approach matters so much.

Why I Look Beyond Hormones

When I work with women, my goal isn’t simply to manage symptoms.

It’s to understand why they’re happening.

That often means looking at how different systems in the body work together.

For example, fluctuating blood sugar can increase inflammation and affect both energy and weight.

Chronic stress can disrupt cortisol, making it harder to think clearly, sleep well, and maintain a healthy metabolism.

Poor gut health may contribute to inflammation throughout the body, including the brain, leaving many women struggling with persistent brain fog despite doing everything “right.”

Every woman is different.

The goal isn’t to assume.

The goal is to understand.

That’s why personalized laboratory testing, when appropriate, can be incredibly valuable. It helps us stop guessing and start making decisions based on what’s actually happening inside your body.

Instead of asking only,

We also ask,

“What’s driving these symptoms?”

I don’t believe that’s true.
